Essenziale Dentali
$399/year
This plan is for individuals who require routine cleanings
- First year: 1 New Patient Comprehensive Exam
- First year: 1 Periodic Exam & Oral Cancer Screening
- Subsequent years: Two exams and oral cancer screening
- 1 Emergency exam with X-rays
- 2 routine cleanings
- 2 Fluoride Treatments
- 1 Full mouth series of X-rays and/or Panoramic X-rays
- One wellness scan
- 20% off all other dental procedures excluding sedation
- 15% off oral surgery treatment at Carolina Oral Surgery & Dental Implant Center (excludes the cost of sedation)
- $500 off clear aligner therapy
- $200 of KöR in-office whitening
- 1 Complimentary smile rejuvenation consult
- An additional cleaning per year can be scheduled for an additional $60. Additional cleanings may be recommended for patients who are pregnant, patients undergoing cancer treatment, or patients with higher rates of cavities.
Dentale Con Più Cura
$650/year
This plan is for individuals who require “deep cleanings” or have an already-diagnosed and treated periodontal disease.
- 1 New Patient Comprehensive Exam
- 1 Periodic Exam & Oral Cancer Screening
- 1 Emergency exam with X-rays
- 4 Periodontal maintenance cleanings
- 2 Fluoride treatments
- 1 Full mouth series and/or Panoramic X-rays
- 1 Wellness scan
- 25% off other Periodontal treatment or therapy
- 20% off all other dental procedures (excludes the cost of sedation)
- 15% off oral surgery treatment with Carolina Oral Surgery & Dental Implant Center (excludes the cost of sedation)
Program Guidelines
- Patient’s portion of the bill is due at the time of services rendered
- There is a $50 reinstatement fee if your plan lapses
- This plan is only honored at Bellissima Dental Boutique and Carolina Oral Surgery & Dental Implant Center
Program Exclusions and Limitations
This program is a discount plan, NOT a dental insurance plan. It cannot be used:
- In conjunction with another dental plan or dental insurance
- For referrals to specialist other than Carolina Oral Surgery & Dental Implant Center
- For costs of dental care which is covered under automobile or medical insurance
- For services of injuries covered under worker’s compensation
